Pre-Admission and admission
We have a pre-admission clinic that must be visited before admission into the hospital. If you cannot visit the pre-admission clinic before your scheduled operation, forms can be collected and faxed through to reception on 011 362 4474 or alternatively emailed to unathi.bangani@lifehealthcare.co.za.
The following items are required for admission:

Service monitoring
In keeping with our goal to continue providing high quality care; we monitor our service to patients on an ongoing basis. On discharge patients are asked to complete a questionnaire regarding their stay at the hospital. Participation is voluntary, but this will assist the hospital management to address any problems and improve our service to you.
We especially welcome any suggestions our patients may have. If you have any suggestions for us, please send us an e-mail at yvette.meyer@lifehealthcare.co.za.

We welcome all visitors, as we know what a difference the support of close family and friends can make to the health and well-being of patients.
